Our providers deliver compassionate, high-quality primary care to everyone, regardless of their ability to pay. Our mission is to ensure that no one in our community goes without the essential healthcare they need and deserve.

EDUCATION & ADVOCACY

Through statewide outreach and health education, we remove barriers to care and expand access to vital prevention and wellness services.

FREE TESTING & REFERRALS

We provide free STI and substance use disorder (SUD) testing, prevention education, and referrals across 40 counties. Our programs reach families where they are.

HOUSING SUPPORT

We offer safe, stable housing and access programs that help individuals recovering from chronic illness or substance use disorder, as well as those reentering society, rebuild their lives with dignity and hope.

WORKFORCE DEVELOPMENT 

We invest in the next generation of community and rural health leaders by offering meaningful outreach, training, and educational opportunities that strengthen Missouri’s healthcare future.

COMMUNITY CONNECTIONS

We connect residents to healthcare, life-skills training, and employment opportunities.

ENROLLMENT ASSISTANCE

Our Certified Application Counselors travel throughout rural Missouri to help individuals and families enroll in Medicaid and Marketplace plans, ensuring everyone has access to affordable healthcare coverage.
